RP Losing Character to Player Associations

Every so often a character will become orphaned of its player; I’m not really sure what to make of it or how to trouble-shoot. There’s nothing going on that I can think of that would cause it. Initially I thought it was an issue with my previous host, but since I’ve moved it’s continued to happen. There doesn’t seem to be any particular character it happens to more frequently than not, but it seems like once a week or so I’ll get a report from a player that they can’t sign up and that’s the culprit.
